I don't know how tall your grid extenders are, but if they are four inches tall like the extra grates I built, they are too tall to use on top of the original top grate. The extra grate I use is 15" in diameter and by the time you put the lid on, there is less than one inch clearance between the edge of the grate and the inside of the lid. Besides that, it cooks a lot hotter in this small area and the air flow is too restricted.
As you said, using an extra 18.5" Kettle grate or an extra top grate for the WSM, turned upside down adds an extra 2" in height instead of four inches or more. It may not be necessary to add the wing nuts if you are going to use a grid extender in the middle. This will allow more air flow between the new lower shelf and the new upper shelf.
