I used to roll it up in foil and then unroll it onto the grill, for the transfer. Then I would use two long spatulas to remove. Last year I started using a grilling rack and it is much simpler. I've done it in a pan too, but I like the texture and 360 degree bark better cooking it like a big fatty.
